**Alisporivir + ribavirin** is an investigational oral combination therapy for hepatitis C virus (HCV) infection. **Alisporivir** is a host-targeting antiviral that inhibits the peptidyl-prolyl isomerase activity of cyclophilin A, a cellular protein essential for HCV replication, conferring pangenotypic anti-HCV activity and a high barrier to resistance. **Ribavirin** is a guanosine analogue antiviral that works through multiple mechanisms, including inhibition of inosine monophosphate dehydrogenase (IMPDH), inducing viral error catastrophe (RNA mutagenesis), inhibition of viral RNA polymerase, and immunomodulation. Clinical studies have shown that the combination (with or without interferon) improves rapid virological response and sustained virological response, especially in HCV genotype 2 or 3, with a favorable safety profile in interferon-free regimens. Alisporivir and ribavirin have not been approved as a fixed combination product; their use in combination is investigational, mainly assessed in clinical trials for HCV.
