Drug intelligence / Profile preview
**ALK7-targeting RNAi + tirzepatide** is an investigational combination therapy being evaluated for the treatment of obesity and metabolic diseases. The combination consists of an RNA interference (RNAi) therapeutic designed to silence the *ACVR1C* gene (which encodes activin receptor-like kinase 7, or ALK7) in adipose tissue, and tirzepatide, a dual glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist. ALK7 is a receptor predominantly expressed in adipocytes that negatively regulates lipolysis and energy homeostasis. Silencing ALK7 promotes lipid mobilization, increases catecholamine sensitivity, and reduces fat mass while preserving lean mass. Tirzepatide, an approved weight-management and type 2 diabetes therapy, reduces body weight primarily by suppressing appetite via the central nervous system. Preclinical and early clinical studies (such as those evaluating Arrowhead Pharmaceuticals' ARO-ALK7 in combination with tirzepatide) suggest that combining these two complementary mechanisms produces synergistic reductions in body weight and fat mass (particularly visceral fat) while completely preserving lean muscle mass, addressing a key limitation of incretin monotherapies.
