Allogeneic adipose-derived mesenchymal stromal cells (ASCs), ex-vivo expanded, is an investigational cell therapy developed by Assistance Publique - Hôpitaux de Paris (AP-HP) for the treatment of life-threatening severe cutaneous adverse reactions (SCARs), including Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN). These cells are harvested from the adipose tissue of healthy donors and expanded under Good Manufacturing Practice (GMP) conditions. ASCs exert their therapeutic effect through potent immunomodulatory and anti-inflammatory mechanisms, primarily by secreting paracrine factors such as interleukin-10 (IL-10), transforming growth factor-beta (TGF-β), and prostaglandin E2 (PGE2), as well as expressing indoleamine 2,3-dioxygenase (IDO). These factors collectively inhibit the activation and proliferation of cytotoxic T-lymphocytes and natural killer cells, which are the primary mediators of keratinocyte apoptosis in SJS/TEN. Additionally, ASCs promote tissue repair and reduce the systemic cytokine storm associated with these conditions. The therapy is currently being evaluated in clinical trials to improve survival and accelerate skin healing in affected patients.
