Allogeneic hematopoietic stem cell transplantation (HSCT) is an investigational cell-based biologic therapy developed by Northwestern University for the treatment of severe, treatment-refractory systemic lupus erythematosus (SLE). The procedure involves a nonmyeloablative conditioning regimen, typically utilizing cyclophosphamide and the anti-CD52 monoclonal antibody alemtuzumab (CAMPATH-1H), to ablate the patient's existing autoreactive immune cells. Following this lymphodepletion, allogeneic hematopoietic stem cells from a healthy donor are infused to repopulate the recipient's immune system. The therapeutic goal of this 'immune reset' is to eliminate the underlying autoimmune pathology and establish a new, donor-derived immune system that is tolerant of the recipient's tissues, potentially leading to long-term clinical remission.
