Overview

This investigational cell therapy approach, developed by the Washington University School of Medicine, utilizes allogeneic hematopoietic stem cells in a sequential transplant strategy for patients with high-risk non-Hodgkin's lymphoma. The protocol involves an initial cytoreductive phase using high-dose chemotherapy followed by an autologous hematopoietic stem cell transplant. Approximately 60 to 120 days later, patients receive a non-myeloablative allogeneic hematopoietic stem cell transplant from HLA-identical related or unrelated donors. The allogeneic phase employs a conditioning regimen of total lymphoid irradiation (TLI) and anti-thymocyte globulin (ATG) to facilitate engraftment and induce a graft-versus-lymphoma (GVL) effect, where donor-derived immune cells target residual malignant cells, while minimizing the toxicities typically associated with myeloablative conditioning.
