**Allogeneic islets + Corline Heparin Conjugate** is a combination therapy where isolated allogeneic pancreatic islets of Langerhans are surface-coated with Corline Heparin Conjugate (CHC) to improve biocompatibility and protect against early graft loss post-transplantation. CHC is a macromolecular conjugate composed of multiple unfractionated heparin chains covalently linked to a polymer backbone[2][3][5]. The coating of islets with CHC mimics the natural vascular endothelium glycocalyx and inhibits activation of coagulation, complement, and inflammatory cascades upon contact with blood, thereby reducing instant blood-mediated inflammatory reaction (IBMIR), thrombosis, and preserving islet function and viability[2][3]. This approach has shown to promote improved engraftment and survival of transplanted islets in experimental settings and is under investigation in applications such as diabetes (type 1) and solid organ transplantation[3][5]. Mechanistically, CHC acts locally at the islet surface as an anticoagulant and anti-inflammatory agent without affecting insulin secretion.
