This combination therapeutic regimen consists of allogeneic mesenchymal stromal cells (MSCs) and extracorporeal photopheresis (ECP). It is currently being evaluated in a Phase II clinical trial led by Molly Gallogly at University Hospitals Cleveland Medical Center for the treatment of high-risk and steroid-refractory acute graft-versus-host disease (aGVHD). Allogeneic MSCs are multipotent cells that exert potent immunomodulatory effects by inhibiting T-cell proliferation and modulating the cytokine environment toward an anti-inflammatory state. ECP is a leukapheresis-based procedure where peripheral blood mononuclear cells are treated ex vivo with a photosensitizing agent and ultraviolet A light to induce apoptosis in pathogenic T-cells and promote immune tolerance. The combination aims to provide synergistic immunosuppression for patients who have failed standard steroid treatments.
