This refers to a standardized allogeneic hematopoietic stem cell transplant (HSCT) protocol developed by the Masonic Cancer Center at the University of Minnesota for the treatment of various hematological malignancies. The procedure involves the infusion of healthy stem cells from a related or unrelated donor (including bone marrow, peripheral blood, or umbilical cord blood) into a patient whose own marrow has been depleted by a conditioning regimen. This conditioning typically includes high-dose chemotherapy (such as cyclophosphamide and busulfan) and/or total body irradiation (TBI). In some variations of the protocol, equine anti-thymocyte globulin (ATG) and the infusion of regulatory T cells (CD4+/CD25+ cells) are utilized to modulate the immune response and reduce the risk of graft-versus-host disease (GVHD). The primary goal is to replace the patient's diseased hematopoietic system with a healthy donor-derived system capable of producing normal blood cells and exerting a graft-versus-tumor effect. The protocol is currently utilized as a standard-of-care treatment guideline at the institution.
