Allogeneic T regulatory cells (Tregs) are an investigational adoptive cell therapy developed by Stanford University for the treatment of acute respiratory distress syndrome (ARDS) induced by COVID-19. Tregs are a specialized subpopulation of T cells that act to suppress immune responses, thereby maintaining homeostasis and self-tolerance. In the context of severe COVID-19, an overactive immune response can lead to a 'cytokine storm' and life-threatening lung injury. This therapy involves isolating Tregs from living related donors (either HLA-matched or haploidentical) via immunomagnetic selection from apheresis products. The cells are then infused intravenously into the patient to dampen excessive pulmonary inflammation and promote tissue repair, with the goal of improving clinical outcomes in ARDS.
