Overview

**Allogeneic umbilical cord mesenchymal stem cells + decitabine** is an experimental combination therapy that pairs allogeneic umbilical cord-derived mesenchymal stem cells (UC-MSCs) with decitabine, a hypomethylating agent. UC-MSCs are stem cells sourced from donated human umbilical cords, possessing immunomodulatory and anti-inflammatory effects, while decitabine is a nucleoside analog that inhibits DNA methyltransferase, leading to hypomethylation of DNA and modulation of gene expression. When combined, decitabine enhances the efficacy of UC-MSCs by promoting polarization of macrophages towards an anti-inflammatory (M2) phenotype in a PPARγ-dependent manner, prolonging and amplifying the anti-diabetic and immunomodulatory effects seen in preclinical models. This approach is under development primarily for conditions such as type 2 diabetes and potentially for improving outcomes of allogeneic stem cell or hematopoietic cell transplantation, including reduction of graft-versus-host disease and improved engraftment[1][2]. The combination leverages the synergistic immune-regulatory mechanisms of both agents, with decitabine acting to epigenetically reprogram immune cells and UC-MSCs contributing tissue repair and immunosuppression.
