Alnuctamab + mezigdomide is an investigational combination therapy currently in clinical trials for relapsed and/or refractory multiple myeloma. Alnuctamab is a bispecific monoclonal antibody that targets both B-cell maturation antigen (BCMA) and CD3, redirecting T cells to kill BCMA-expressing myeloma cells. Mezigdomide is a small molecule cereblon E3 ligase modulator (CELMoD) that induces targeted ubiquitination and proteasomal degradation of specific transcription factors (notably IKZF1/Ikaros and IKZF3/Aiolos), leading to immunomodulatory and antineoplastic effects. Preclinical studies demonstrate mezigdomide enhances alnuctamab-mediated T-cell cytotoxicity by increasing immune cell–tumor interactions and T-cell activation. The combination is being assessed for safety, tolerability, and preliminary efficacy in patients previously treated for multiple myeloma, including those with resistance to prior lines of therapy[1][3][5][7][9].
