Alpha blocker + solifenacin is a combination therapeutic regimen used for the treatment of residual overactive bladder (OAB) symptoms in men who have not achieved adequate relief with alpha blocker monotherapy. Solifenacin is a competitive muscarinic receptor antagonist with selectivity for M1 and M3 receptor subtypes, which are primarily responsible for bladder detrusor muscle contraction. Alpha blockers (alpha-1 adrenergic receptor antagonists) are standard treatments for lower urinary tract symptoms (LUTS) associated with benign prostatic hyperplasia (BPH), working by relaxing smooth muscle in the bladder neck and prostate. The combination therapy aims to address both storage symptoms (urgency, frequency) via solifenacin and voiding symptoms via alpha blockers. This specific combination was evaluated in a Phase 4 randomized, double-blind study (NCT00950300) led by investigator Kyu-Sung Lee to determine the efficacy and safety of adding solifenacin to stable alpha blocker therapy.
