Overview

This investigational combination regimen is being developed by BioVersys AG for the treatment of tuberculosis meningitis (TBM). It consists of **AlpE** (a combination of the potentiator **alpibectir** and the antibiotic **ethionamide**) administered alongside the standard first-line tuberculosis treatment regimen, commonly known as **HRZE** (isoniazid, rifampin, pyrazinamide, and ethambutol). Alpibectir is a small molecule developed from BioVersys' Transcriptional Regulatory Inhibitory Compounds (TRIC) platform. It acts as a transcriptional regulator inhibitor that specifically targets the EthR repressor in *Mycobacterium tuberculosis*. By inhibiting EthR, alpibectir increases the bioactivation of ethionamide, thereby overcoming resistance and enhancing its bactericidal activity. The regimen is designed to improve outcomes in TBM, a severe form of tuberculosis with high mortality and morbidity, by providing rapid bactericidal activity and better penetration into the central nervous system. It is currently being evaluated in Phase 2a clinical trials to assess safety, tolerability, and pharmacokinetics in both plasma and cerebrospinal fluid.
