Overview
alteplase + edaravone is a combination therapy being evaluated for the treatment of acute ischemic stroke (AIS). This regimen combines the thrombolytic agent alteplase (a recombinant tissue plasminogen activator) with edaravone, a neuroprotective free radical scavenger. In the Phase 4 EXCELLENT trial led by Ruijun Ji, this combination is administered ultra-early in a mobile stroke unit (MSU) setting. Alteplase is delivered intravenously to dissolve the causative blood clot by converting plasminogen to plasmin, while edaravone is administered sublingually to mitigate oxidative stress and neuronal damage associated with ischemia and subsequent reperfusion. The study specifically evaluates whether this early neuroprotection can improve clinical outcomes and reduce hemorrhagic transformation following revascularization.
