Overview

Alteplase + tenecteplase is a hypothetical combination of two recombinant tissue plasminogen activators (rt-PA) used as thrombolytic agents. Both drugs act by catalyzing the conversion of plasminogen to plasmin, leading to the breakdown of fibrin clots. Alteplase is a first-generation rt-PA approved for acute ischemic stroke (AIS), acute myocardial infarction (AMI), and pulmonary embolism (PE). Tenecteplase is a genetically engineered variant of alteplase with three amino acid substitutions, resulting in higher fibrin specificity, greater resistance to plasminogen activator inhibitor-1, longer half-life, and single bolus administration. Tenecteplase has regulatory approval for AMI and is increasingly used off-label for AIS due to evidence suggesting noninferiority or potential superiority over alteplase in certain settings such as large vessel occlusion[1][2][5]. There are no known clinical products or guidelines recommending simultaneous use of both agents together.
