Alteplase + urokinase is a combination of two thrombolytic (fibrinolytic) agents used to dissolve blood clots. Alteplase is a recombinant form of human tissue plasminogen activator (tPA), which binds to fibrin in a clot and converts plasminogen to plasmin, leading to the breakdown of fibrin and dissolution of the clot[1][4]. Urokinase is an endogenous serine protease that directly converts plasminogen to plasmin, promoting fibrinolysis independently of fibrin binding[5]. Both drugs are indicated individually for conditions such as acute ischemic stroke, myocardial infarction, pulmonary embolism, and catheter clearance. Combination regimens have been explored in clinical trials for enhanced efficacy or safety in thrombolytic therapy but are not standard practice or widely approved as a fixed-dose combination[3]. The mechanism involves synergistic activation of the body's natural clot-dissolving pathways.
