Overview

Alum-ALM + BCG is a combination vaccine therapy primarily investigated for leishmaniasis treatment and prevention. It consists of alum-precipitated autoclaved Leishmania major (Alum-ALM) mixed with Bacille Calmette-Guerin (BCG). ## Composition and Mechanism Alum-ALM + BCG combines two key components: 1. **Alum-ALM**: This is aluminum hydroxide (alum) precipitated autoclaved Leishmania major, which serves as the primary antigen in the vaccine formulation. 2. **BCG**: Bacille Calmette-Guerin, a vaccine originally developed against tuberculosis, is used as an immunostimulant adjuvant in this combination. The mechanism of action involves stimulating a strong Th1 immune response while inhibiting Th2 responses, which is crucial for combating intracellular infections like Leishmania. When administered, this combination induces delayed-type hypersensitivity (DTH) reactions and promotes cellular immunity. ## Clinical Applications The combination has been studied for several applications: - **Post-Kala-Azar Dermal Leishmaniasis (PKDL)**: In a randomized, double-blind study in Sudan, Alum-ALM + BCG was used alongside sodium stibogluconate (SSG) as an immunochemotherapeutic approach. This regimen was found to be safe, immunogenic, and demonstrated significant healing potential in persistent PKDL lesions. - **Visceral Leishmaniasis (VL)**: Clinical studies have shown that the approach was safe and could induce strong delayed-type hypersensitivity reactions in study volunteers. - **Canine Visceral Leishmaniasis (CVL)**: A double-blind randomized trial tested Alum-ALM mixed with BCG plus imiquimod against CVL. The vaccine was found to be safe with only one case showing side effects. It produced strong skin test conversion in the vaccinated group compared to the control group, though the overall efficacy based on seroconversion was relatively low at 40.4%. ## Safety Profile Multiple studies have confirmed the safety of this combination: - Three injections of BCG produced no observable adverse reactions, suggesting it could be used in booster injections to increase protective potential. - In PKDL treatment, the main side effects reported were injection site indurations and ulcers, and myalgia. - In the CVL trial, there were minimal side effects associated with vaccination except for one case. The combination appears to be well-tolerated overall, making it suitable for consideration in various leishmaniasis treatment and prevention strategies.
