Ambroxol + ipratropium is a combination therapy consisting of a mucolytic agent and a short-acting muscarinic antagonist (SAMA), primarily utilized in the management of respiratory secretions and airway obstruction in critically ill patients. Ambroxol acts as a secretolytic and mucolytic agent; it thins bronchial secretions and stimulates the production of pulmonary surfactant by alveolar type II cells, thereby facilitating mucus clearance and improving gas exchange. Ipratropium bromide is a quaternary ammonium derivative that acts as a competitive antagonist at muscarinic acetylcholine receptors (M1, M2, and M3) in the bronchial smooth muscle, leading to potent bronchodilation. This combination is particularly effective in patients undergoing mechanical ventilation, where it helps to reduce airway resistance, prevent mucus plugging, and decrease the incidence of ventilator-associated complications. Clinical research, including studies conducted at the Eastern Hepatobiliary Hospital in Shanghai, has demonstrated that the synergistic action of these two agents can improve respiratory mechanics and potentially shorten the duration of mechanical ventilation.
