The combination of amifostine, cisplatin, and vinblastine (often referred to as the ACV regimen) is a multi-agent chemotherapy protocol primarily investigated for metastatic non-small cell lung cancer (NSCLC). Amifostine is a cytoprotective agent that acts as a prodrug, selectively protecting normal tissues from the toxic effects of chemotherapy and radiation by scavenging free radicals and donating thiol groups. This combination has shown promising response rates in phase II trials for metastatic NSCLC but carries risks of significant hematologic toxicity (notably neutropenia), nephrotoxicity (generally reversible), neuropathy, ototoxicity from cisplatin, and hypotension related to amifostine administration[2][3].
