This intervention involves the intravenous infusion of a balanced mixture of amino acids, specifically investigated for the prevention of acute kidney injury (AKI) in patients undergoing cardiac surgery with cardiopulmonary bypass. Developed and studied by Università Vita-Salute San Raffaele and the IRCCS San Raffaele Scientific Institute, the therapy is administered at a dose of 2 g per kilogram of ideal body weight per day. The mechanism of action involves the recruitment of renal functional reserve, which leads to an increase in renal blood flow and the glomerular filtration rate through afferent arteriolar dilation. The Phase III PROTECTION trial demonstrated that a continuous infusion of these amino acids from the start of surgery up to 72 hours postoperatively significantly reduced the incidence of AKI by 15% compared to a placebo, providing high-level evidence for this prophylactic strategy.
