Amino acid + glucose intradialytic parenteral nutrition (IDPN) is a specialized intravenous nutritional support therapy administered to patients with end-stage kidney disease (ESKD) during their hemodialysis sessions. This intervention is designed to combat protein-energy wasting (PEW) and muscle protein turnover abnormalities, which are common complications in chronic dialysis patients due to the catabolic nature of the procedure and often inadequate oral intake. The formulation typically consists of a concentrated mixture of essential and non-essential amino acids along with glucose (dextrose), providing both the building blocks for protein synthesis and an energy source to spare protein from oxidation. By delivering these nutrients directly into the venous drip chamber of the dialysis circuit, IDPN bypasses the gastrointestinal tract and counteracts the nutrient losses that occur during dialysis. The LOTUS trial, conducted by Erasmus MC, specifically evaluates the efficacy of this nutritional strategy in improving muscle protein synthesis and amino acid balance in chronic hemodialysis patients.
