Aminoleban + l-ornithine l-aspartate is a combination therapy being investigated for the treatment of hepatic encephalopathy (HE), particularly in critically ill patients. Aminoleban is a specialized parenteral nutrition solution enriched with branched-chain amino acids (BCAA)—leucine, isoleucine, and valine—designed to correct the amino acid imbalances associated with liver failure and provide substrates for ammonia detoxification in skeletal muscle. L-ornithine L-aspartate (LOLA) is a stable salt of two natural amino acids that lowers blood ammonia levels by stimulating the urea cycle in the liver and promoting glutamine synthesis in both the liver and muscle. By combining these two agents, the therapy aims to synergistically enhance ammonia clearance and improve neurological outcomes in patients with Grade III or IV hepatic encephalopathy.
