Overview

Aminophylline + dexamethasone is a combination therapy being investigated for the prevention and reduction of severity of post dural puncture headache (PDPH). Aminophylline is a methylxanthine derivative that acts as a non-selective adenosine receptor antagonist and phosphodiesterase inhibitor; in the context of PDPH, it is thought to counteract adenosine-mediated cerebral vasodilation, thereby inducing vasoconstriction and potentially stimulating cerebrospinal fluid production. Dexamethasone is a potent synthetic glucocorticoid with significant anti-inflammatory properties that may reduce dural inflammation and edema at the site of the puncture. This combination is typically administered as a slow intravenous infusion in surgical patients, particularly those with a history of PDPH, to mitigate the risk of headache following spinal anesthesia.
