Aminophylline + isoprenaline is a combination of two bronchodilator drugs used primarily in the management of bronchial asthma and other obstructive airway diseases. Aminophylline, a methylxanthine derivative, acts mainly as a non-selective phosphodiesterase inhibitor leading to increased intracellular cyclic AMP levels, resulting in relaxation of bronchial smooth muscle. It also antagonizes adenosine receptors. Isoprenaline (also known as isoproterenol) is a non-selective beta-adrenergic agonist that stimulates both beta-1 and beta-2 adrenergic receptors, causing bronchodilation and positive cardiac effects. The combination has been shown to act synergistically in improving airflow parameters in asthma by increasing maximum expiratory flow rates more than either drug alone at certain time points[1][2]. Both agents also have pulmonary vasodilator properties[4]. This combination may be considered when rapid bronchodilation or enhanced effect over monotherapy is desired.
