Overview

Aminophylline + salbutamol is a combination of two bronchodilator drugs used primarily in the management of acute asthma and chronic obstructive pulmonary disease (COPD). Aminophylline is a methylxanthine derivative that acts as a non-selective phosphodiesterase inhibitor, leading to increased intracellular cyclic AMP and subsequent relaxation of bronchial smooth muscle. It also antagonizes adenosine receptors, contributing to its bronchodilatory effects. Salbutamol (also known as albuterol) is a selective beta-2 adrenergic receptor agonist that stimulates beta-2 receptors in bronchial smooth muscle, resulting in rapid bronchodilation. The combination may be administered intravenously or via nebulization for acute exacerbations when monotherapy is insufficient. However, clinical studies have shown no significant synergistic benefit over aminophylline alone for acute asthma treatment; side effects such as tachycardia may be more common with the combination[1][2][3][5]. Both agents are widely available and inexpensive options for emergency respiratory care.
