Amiodarone + placebo refers to the control arm of the Adjunctive Ventricular Arrhythmia Suppression Trial (AVAST) Pilot, a clinical study sponsored by Hartford Hospital. This regimen involves the administration of a stable dose of amiodarone, a potent Class III antiarrhythmic agent, in combination with an inert placebo. Amiodarone is used to manage life-threatening ventricular arrhythmias by prolonging the cardiac action potential and refractory period. Its complex mechanism of action includes the blockade of potassium channels (primarily the rapid delayed rectifier current, IKr), sodium channels, and L-type calcium channels, as well as non-competitive antagonism of alpha- and beta-adrenergic receptors. In the AVAST Pilot study, this combination served as the comparator against the experimental arm of amiodarone plus ranolazine to evaluate potential pharmacokinetic interactions and synergistic effects on the QT interval and arrhythmia suppression in patients with implantable cardioverter defibrillators (ICDs).
