Amiodarone + sotalol is a combination of two antiarrhythmic drugs, both classified as class III antiarrhythmics. Amiodarone primarily works by blocking potassium channels, prolonging repolarization and the refractory period in cardiac tissue, which helps prevent abnormal heart rhythms. Sotalol acts as both a non-selective beta-adrenergic receptor blocker (class II) and a potassium channel blocker (class III), reducing heart rate and also prolonging ventricular repolarization. This combination has been used off-label for refractory ventricular tachycardia or atrial fibrillation when monotherapy is insufficient. However, combining these agents significantly increases the risk of serious arrhythmias such as QT interval prolongation and torsade de pointes; therefore, this regimen should only be considered under special circumstances with close monitoring[1][2][4][5].
