Amitriptyline + Ketamine

Amitriptyline + ketamine is a combination of two pharmaceutical agents used primarily for the treatment of neuropathic pain. Amitriptyline is a tricyclic antidepressant that inhibits the reuptake of serotonin and noradrenaline, increasing their levels in the central nervous system and blocking sodium channels, which contributes to its analgesic effects. Ketamine is a general anesthetic that acts as an antagonist at N-methyl-D-aspartate (NMDA) receptors, reducing pain by blocking these receptors on peripheral nociceptive afferents and possibly modulating other glutamatergic receptors such as AMPA and kainate. The combination has been studied mainly in topical formulations for conditions like peripheral neuropathic pain, pelvic pain, rectal/genital/perineal pain syndromes, showing moderate to substantial long-term reduction in perceived pain with minimal systemic absorption or adverse effects[1][3][4][5][8].
