Overview

Amitriptyline + tranylcypromine is a combination of two antidepressant medications used off-label for treatment-resistant major depressive disorder, particularly in patients who have not responded to electroconvulsive therapy (ECT). Amitriptyline is a tricyclic antidepressant that inhibits the reuptake of serotonin and norepinephrine, increasing their synaptic concentrations. Tranylcypromine is a nonselective, irreversible monoamine oxidase inhibitor (MAOI) that prevents the breakdown of monoamines such as serotonin, norepinephrine, and dopamine. The combination aims to enhance antidepressant efficacy by targeting multiple mechanisms involved in mood regulation. While some studies suggest this combination can be effective and relatively well-tolerated under close supervision for ECT-resistant depression, it carries significant risks—including hypertensive crisis and serotonin syndrome—due to pharmacodynamic interactions. This regimen should only be considered when other treatments have failed and with careful monitoring[1][2][5].
