Amlodipine + folic acid + 5-methyltetrahydrofolate is a combination drug consisting of a calcium channel blocker (amlodipine), synthetic folic acid, and the bioactive, naturally occurring folate 5-methyltetrahydrofolate (5-MTHF). Amlodipine inhibits the influx of calcium ions into vascular smooth muscle and cardiac muscle, producing an antihypertensive and antianginal effect by relaxing blood vessels. Folic acid and 5-MTHF are forms of vitamin B9: folic acid is a synthetic precursor requiring enzymatic reduction, whereas 5-MTHF is the main circulating and physiological form, immediately bioavailable and critical for methylation/metabolic cycles. The combination is intended to lower blood pressure and reduce homocysteine levels, particularly in patients with hypertension and hyperhomocysteinemia, and is sometimes considered for patients with MTHFR genetic polymorphisms affecting folate metabolism[1][2][6].
