amlodipine besilate + indapamide + perindopril erbumine is a fixed-dose combination (FDC) therapy developed by Zentiva (under the Zent2U brand) for the management of hypertension. This small molecule combination integrates three antihypertensive agents with distinct and synergistic mechanisms of action: amlodipine, a dihydropyridine calcium channel blocker (CCB); indapamide, a thiazide-like diuretic; and perindopril, an angiotensin-converting enzyme (ACE) inhibitor. Amlodipine lowers blood pressure by inhibiting the influx of calcium ions into vascular smooth muscle, leading to vasodilation. Indapamide promotes the excretion of sodium and water by inhibiting the sodium-chloride symporter in the distal convoluted tubule of the kidney. Perindopril, administered as the erbumine salt, is a prodrug that is converted to perindoprilat, which inhibits ACE, thereby reducing the production of angiotensin II and decreasing systemic vascular resistance. This triple therapy is typically indicated for patients whose hypertension is not adequately controlled by dual therapy or as a substitution for patients already stabilized on the individual components.
