Amoxicillin + clarithromycin + bismuth is a combination therapy primarily utilized as the core of bismuth-containing quadruple therapy (BQT) for the eradication of *Helicobacter pylori*. This regimen is particularly significant in clinical practice in China, where it is recommended as a first-line treatment by national consensus guidelines, with extensive research and validation conducted by institutions such as Peking University First Hospital. Amoxicillin, a beta-lactam antibiotic, inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins. Clarithromycin, a macrolide, prevents bacterial protein synthesis by binding to the 50S ribosomal subunit. Bismuth salts (such as bismuth potassium citrate) provide a synergistic effect by exerting direct antimicrobial activity, inhibiting the urease enzyme, and preventing bacterial adhesion to the gastric mucosa. This multi-pronged approach is effective in overcoming bacterial resistance to individual antibiotics and enhancing overall eradication rates.
