Amoxicillin + gentamicin + metronidazole is a combination antibiotic regimen consisting of three generic antimicrobial agents used together to treat uncomplicated acute appendicitis. Amoxicillin is a beta-lactam antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, targeting gram-positive bacteria. Gentamicin is an aminoglycoside that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, targeting gram-negative bacteria. Metronidazole is a nitroimidazole that disrupts bacterial DNA, targeting anaerobic bacteria. This triple-therapy protocol provides broad-spectrum coverage against the polymicrobial flora typically implicated in appendicitis. It has been evaluated in Phase 3 clinical trials, such as those sponsored by the University of Queensland, as a non-surgical alternative to laparoscopic appendicectomy in adults with radiologically confirmed uncomplicated disease.
