ampicillin + amoxicillin + azithromycin is a combination antibiotic regimen currently being investigated by Inova Health Care Services as a latency antibiotic strategy for the management of preterm prelabor rupture of membranes (PPROM). This therapeutic approach is specifically targeted at pregnant patients diagnosed with PPROM between 22 and 32 weeks of gestation, with the primary goal of prolonging the pregnancy latency period until at least 34 weeks to mitigate the risks of neonatal morbidity associated with prematurity. The regimen involves an initial course of intravenous ampicillin and oral azithromycin, followed by oral amoxicillin and azithromycin. Azithromycin, a macrolide antibiotic, works by inhibiting bacterial protein synthesis through binding to the 50S ribosomal subunit. Ampicillin and amoxicillin are beta-lactam antibiotics that inhibit bacterial cell wall synthesis by targeting penicillin-binding proteins (PBPs). This combination is being evaluated in a cluster randomized comparative effectiveness trial as a more tolerable, easier-to-administer, and more accessible alternative to the traditional erythromycin-based latency regimen, particularly in the context of national erythromycin shortages.
