ampicillin + ceftriaxone

This is a combination antibiotic therapy consisting of ampicillin, a penicillin-class beta-lactam, and ceftriaxone, a third-generation cephalosporin. It is being investigated by the Fundación Pública Andaluza para la gestión de la Investigación en Sevilla in the Phase IV DOBLEI trial for the treatment of infective endocarditis caused by Enterococcus faecalis. The regimen exploits the synergistic effect of these two beta-lactams, where ceftriaxone saturates penicillin-binding proteins (PBPs) 2 and 3, enhancing the activity of ampicillin against PBP 4 and 5 in E. faecalis. The trial specifically evaluates the efficacy and safety of continuous intravenous infusion compared to standard intermittent dosing to potentially shorten hospital stays from 4-6 weeks to 2-3 weeks.
