ampicillin + penicillin refers to a combination or alternative regimen of beta-lactam antibiotics used for the treatment of childhood pneumonia, particularly in resource-limited settings. Ampicillin is an aminopenicillin with broad-spectrum activity against Gram-positive and some Gram-negative bacteria, while penicillin (typically referring to benzylpenicillin or Penicillin G) is a narrow-spectrum antibiotic effective against Gram-positive organisms like *Streptococcus pneumoniae*. Both drugs function by binding to penicillin-binding proteins (PBPs), which are enzymes involved in the synthesis of peptidoglycan in the bacterial cell wall. By binding covalently and irreversibly to the active site of these enzymes, the antibiotics inhibit the transpeptidation reaction, preventing the cross-linking of peptidoglycan chains and leading to bacterial cell wall weakening and osmotic lysis. In the context of the ITIP3 (Innovative Treatments in Pneumonia) study led by Save the Children in Malawi, these parenteral antibiotics were evaluated as part of the standard of care for children aged 2 to 59 months presenting with pneumonia and high-risk comorbidities such as HIV, malnutrition, or malaria.
