Ampicillin + procaine is a combination pharmaceutical consisting of the aminopenicillin antibiotic ampicillin and the local anesthetic procaine. Ampicillin is a beta-lactam antibiotic that exerts its bactericidal activity by binding to penicillin-binding proteins (PBPs) located on the inner membrane of the bacterial cell wall. This binding inhibits the transpeptidation enzymes responsible for cross-linking peptidoglycan chains, leading to cell wall instability and osmotic lysis. Procaine is an ester-type local anesthetic that is typically included in injectable formulations to mitigate the pain associated with intramuscular administration. It functions by binding to and inhibiting voltage-gated sodium channels on the neuronal membrane, thereby preventing the initiation and conduction of nerve impulses. This combination is primarily utilized in veterinary medicine for the treatment of systemic infections caused by susceptible Gram-positive and Gram-negative bacteria, where the addition of procaine facilitates the administration of large-volume doses.
