Anc80L65 + trastuzumab is an investigational **AAV gene therapy construct encoding trastuzumab** that uses the synthetic ancestral adeno-associated virus capsid Anc80L65 to deliver genetic material for local production of the anti-HER2 monoclonal antibody trastuzumab within the central nervous system. The program was developed by Affinia Therapeutics as a one-time lumbar puncture administered therapy intended primarily for **HER2-positive breast cancer brain metastases**, with the goal of overcoming the limited practicality and distribution of conventional systemic trastuzumab in the CNS. Mechanistically, the therapeutic effect is mediated by transgene-derived trastuzumab binding to **human epidermal growth factor receptor 2**, thereby blocking HER2 signaling and promoting antitumor immune effector activity against HER2-expressing tumor cells.
