The combination of androgen deprivation therapy (ADT) and 177Lu-PSMA-617 (lutetium Lu 177 vipivotide tetraxetan) is a therapeutic approach for the treatment of prostate cancer. 177Lu-PSMA-617, developed by Novartis and marketed as Pluvicto, is a targeted radioligand therapy that delivers beta-minus radiation directly to cells expressing prostate-specific membrane antigen (PSMA), which is highly overexpressed in prostate cancer. The therapy consists of the radionuclide lutetium-177 conjugated to the PSMA-targeting ligand vipivotide via a tetraxetan chelator. ADT is a standard-of-care hormonal therapy that reduces circulating androgen levels to inhibit tumor growth. This combination is being evaluated across multiple disease stages, including a Phase II trial at the Sidney Kimmel Comprehensive Cancer Center at Johns Hopkins for high-risk localized prostate cancer, where it is administered in conjunction with external beam radiation therapy (EBRT). The synergy between systemic hormonal suppression and targeted radiopharmaceutical-induced DNA damage aims to improve clinical outcomes and potentially allow for shorter courses of ADT.
