Angiotensin II + hydrocortisone sodium succinate is a combination of two agents used primarily in the management of distributive shock (such as septic shock) and other forms of vasodilatory or refractory hypotension. Angiotensin II is an endogenous peptide hormone that acts as a potent vasoconstrictor by binding to angiotensin II type 1 receptors on vascular smooth muscle, leading to increased blood pressure. It is approved for use as a vasopressor in patients with septic or other distributive shock who do not respond adequately to conventional therapies[5]. Hydrocortisone sodium succinate is a synthetic corticosteroid prodrug rapidly converted in vivo to active hydrocortisone. It exerts anti-inflammatory and immunosuppressive effects by binding to glucocorticoid receptors and modulating gene expression; it also has mineralocorticoid activity and can help stabilize blood pressure in critical illness[2][3][4]. The combination may be used when rapid hemodynamic stabilization is required due to severe inflammation and refractory hypotension.
