Anisodamine hydrobromide + heparin is a fixed-dose combination of two agents: anisodamine hydrobromide, an anticholinergic and vasodilatory small molecule primarily used in China for circulatory disorders such as septic shock, and heparin, an established anticoagulant. The rationale for combining these drugs is based on their complementary mechanisms—anisodamine improves microcirculation by antagonizing muscarinic acetylcholine receptors (mAChRs), reducing vasospasm and improving blood flow, while heparin prevents thrombosis by inhibiting clotting factors. Clinical studies suggest that this combination can reduce early thrombosis risk after arteriovenous fistula creation in chronic kidney disease patients and may be beneficial in sepsis or septic shock by improving vascular patency and reducing mortality[2][3][4]. Anisodamine’s additional properties include weak alpha(1) adrenergic antagonism, anti-thrombotic effects via thromboxane synthesis inhibition, antioxidant activity, and protection against arrhythmia[6]. Heparin acts as an indirect inhibitor of thrombin (factor IIa) and factor Xa through antithrombin III activation.
