Overview

The term "anti-PD-L1 + chemotherapy" refers to a therapeutic regimen combining an anti-programmed death-ligand 1 (PD-L1) monoclonal antibody with standard cytotoxic chemotherapy. Anti-PD-L1 antibodies, such as atezolizumab, durvalumab, and avelumab, are immune checkpoint inhibitors that block the interaction between PD-1 on T cells and PD-L1 on tumor or immune cells. This blockade enhances T cell-mediated antitumor immunity by preventing tumor-induced immune suppression. Chemotherapy induces direct cytotoxicity against rapidly dividing cancer cells and can also increase tumor antigen release, potentially enhancing the immunogenicity of tumors and synergizing with immunotherapy. This combination has become a standard first-line treatment for several advanced cancers—most notably non-small-cell lung cancer (NSCLC)—where it has demonstrated improved progression-free survival (PFS), overall survival (OS), and objective response rates compared to chemotherapy alone[1][6]. The approach is under investigation or approved for other malignancies as well.
