This is a combination immunotherapy regimen, known as the DIPIT protocol, being investigated for the treatment of new-onset type 1 diabetes (T1D). Developed by investigators Camillo Ricordi and Jay Skyler at the University of Miami, the regimen consists of four agents: anti-thymocyte globulin (ATG), low-dose interleukin-2 (IL-2), adalimumab, and exenatide. The strategy employs a "reset and protect" approach: ATG depletes autoreactive T cells, low-dose IL-2 expands regulatory T cells (Tregs) to restore immune tolerance, adalimumab (a TNF inhibitor) reduces systemic and local inflammation, and exenatide (a GLP-1 receptor agonist) supports pancreatic beta-cell function and survival. The primary objective of this multi-drug intervention is to preserve endogenous insulin production in patients recently diagnosed with T1D.
