Overview

This is a combination therapy consisting of anti-thymocyte globulin (ATG), a polyclonal antibody preparation that targets human T lymphocytes and induces immunosuppression, and pegylated granulocyte colony-stimulating factor (pegylated G-CSF or pegfilgrastim), a long-acting recombinant growth factor that stimulates the production and function of neutrophils. The combination has been studied primarily for its potential to preserve pancreatic β cell function in patients with established type 1 diabetes (T1D). Mechanistically, ATG depletes pathogenic T cells while sparing regulatory T cells (Tregs), whereas pegylated G-CSF mobilizes hematopoietic stem cells and further enhances immune regulation by increasing Treg frequency. Clinical studies have shown that this regimen can slow the decline of C-peptide levels—a marker of endogenous insulin secretion—and may help maintain glycemic control in new-onset or established T1D. The approach leverages synergistic immunomodulation to alter disease progression[1][2][3][4][5].
