Overview

Atorvastatin is a small molecule HMG-CoA reductase inhibitor (statin) being investigated as an adjunct therapy to standard antiretroviral therapy (ART) in HIV-infected adults. While its primary mechanism is the inhibition of cholesterol biosynthesis, it is utilized in this context for its pleiotropic immunomodulatory effects, specifically its ability to reduce T-cell immune activation and systemic inflammation. This approach aims to mitigate the chronic inflammatory state associated with HIV infection, which persists even under effective viral suppression. The STAR trial, led by Makerere University, evaluates the equivalence of atorvastatin and rosuvastatin in reducing these markers of immune aging and inflammation in Sub-Saharan African populations.
