Reduced ATG + mini PTCy is a combination graft-versus-host disease (GVHD) prophylaxis regimen consisting of reduced-dose anti-thymocyte globulin (ATG) and mini-dose post-transplant cyclophosphamide (PTCy). This regimen is primarily investigated for use in haploidentical hematopoietic stem cell transplantation (haplo-HSCT) for patients with hematologic malignancies such as acute leukemia and myelodysplastic syndromes (MDS). ATG is a polyclonal antibody preparation that depletes T-cells in vivo, while cyclophosphamide is an alkylating agent administered post-transplant to selectively eliminate proliferating alloreactive T-cells. The combination aims to synergistically reduce the incidence of both acute and chronic GVHD while potentially improving immune reconstitution and reducing infection-related mortality compared to standard-dose ATG protocols.
