**Antroquinonol + nab-paclitaxel + gemcitabine** is an investigational oral/IV combination regimen being developed as a first-line treatment for metastatic pancreatic cancer. This regimen includes: - **Antroquinonol**, a small molecule natural product (ubiquinone derivative) from Antrodia camphorata, which acts primarily as a PI3K/Akt/mTOR pathway inhibitor and AMP-activated protein kinase (AMPK) activator, disrupting cancer cell proliferation and inducing apoptosis and autophagy. - **Nab-paclitaxel** (nanoparticle albumin-bound paclitaxel), a chemotherapy agent that promotes microtubule stabilization inhibiting mitosis. - **Gemcitabine**, a cytidine analog inhibiting DNA synthesis, widely used as a standard care backbone in pancreatic cancer. Clinical data show that the combination leads to a significant survival benefit (median overall survival up to 14.1 months vs. 8.5 months for standard gemcitabine/nab-paclitaxel), with a manageable safety profile and reduced hematologic adverse events compared to the chemotherapy doublet. The regimen is currently in Phase II trials and has received Orphan Drug Designation for pancreatic cancer[2][3][4].
