Apalutamide + docetaxel is a combination therapy investigated for the treatment of metastatic hormone-sensitive prostate cancer (mHSPC), typically administered in conjunction with androgen deprivation therapy (ADT) as a triplet regimen. Apalutamide is a second-generation selective androgen receptor (AR) antagonist that works by binding directly to the ligand-binding domain of the AR, thereby inhibiting its nuclear translocation, DNA binding, and transcription of androgen-responsive genes. Docetaxel is a cytotoxic taxane that promotes the assembly of tubulin into stable microtubules and inhibits their disassembly, leading to cell cycle arrest in the G2/M phase and subsequent apoptosis. This combination is particularly studied in patients with high-volume or high-burden metastatic disease to improve clinical outcomes such as progression-free survival and overall survival compared to standard doublet therapies.
