Apatinib + sintilimab + irinotecan + paclitaxel is a combination therapy regimen being evaluated for the treatment of advanced gastric cancer and gastroesophageal junction adenocarcinoma. The regimen consists of apatinib, a small molecule tyrosine kinase inhibitor that selectively targets vascular endothelial growth factor receptor 2 (VEGFR-2) to inhibit tumor angiogenesis; sintilimab, a monoclonal antibody that blocks the programmed cell death protein 1 (PD-1) receptor to restore anti-tumor immune activity; and cytotoxic chemotherapy, either irinotecan (a topoisomerase I inhibitor) or paclitaxel (a microtubule stabilizer). This multi-modal approach, investigated in a Phase II study at Tianjin Medical University Cancer Institute and Hospital, aims to leverage the synergistic effects of anti-angiogenic therapy, immune checkpoint inhibition, and traditional chemotherapy in patients who have failed prior treatments.
