This combination therapy regimen is being investigated as a first-line treatment for metastatic intrahepatic cholangiocarcinoma (ICC) in clinical trials led by Sun Yat-sen University. The regimen integrates the anti-angiogenic tyrosine kinase inhibitor apatinib, the PD-1-targeting monoclonal antibody sintilimab, and the FOLFOX chemotherapy backbone (comprising oxaliplatin, 5-fluorouracil, and leucovorin). Apatinib selectively inhibits VEGFR-2 to disrupt tumor blood supply, while sintilimab restores T-cell activity against tumor cells by blocking the PD-1/PD-L1 checkpoint. The addition of FOLFOX provides direct cytotoxic effects through DNA alkylation and thymidylate synthase inhibition, potentially enhancing the immunogenicity of the tumor and leading to synergistic anti-tumor activity in advanced biliary tract cancers.
